Minipuberty
A transient post-natal activation of the HPG axis from birth through about six months. The axis behaves almost as it will at puberty proper — and what it does in this brief window has lifelong consequences.
The trigger
Section titled “The trigger”In utero, the high circulating placental sex steroids suppress the fetal hypothalamus by negative feedback. At birth, those steroids are removed within hours, and the infant hypothalamus — released from suppression — briefly resumes pulsatile GnRH release.
The hormone profile
Section titled “The hormone profile”For the first few months of life:
- LH and FSH rise nearly to pubertal levels.
- Testosterone surges to mid-pubertal levels (perhaps 250 ng/dL), well above what will be seen again until puberty proper.
- Inhibin B and AMH are elevated, reflecting active Sertoli-cell function.
By about six months, central inhibition reasserts itself and the axis goes quiet again — and stays quiet for years.
Why it matters
Section titled “Why it matters”Minipuberty is not a curiosity; it has functional outputs that are not made up later.
- Completes testicular descent if the third-trimester fetal burst did not finish the job.
- Supports normal newborn penile growth.
- Drives Sertoli-cell proliferation. This is the critical one. Sertoli-cell number is set during this window and barely changes afterwards. Adult sperm-production capacity is proportional to Sertoli-cell number — so a child who misses minipuberty (severe congenital HPG dysfunction) has a permanently lower ceiling on fertility, even if puberty is later induced with hormones.
Diagnostic window
Section titled “Diagnostic window”Minipuberty also offers a rare opportunity. Outside of this window and outside of puberty proper, the HPG axis is too quiet to evaluate: an infant or young child with congenital hypogonadism will have low LH, FSH, and testosterone — which is normal for that age. During minipuberty the axis is active, so the failure to activate is detectable. In infants with cryptorchidism or micropenis, measuring LH, FSH, and testosterone in the first months of life can detect congenital hypogonadism years before puberty would otherwise reveal it.
Key idea
Section titled “Key idea”Minipuberty is the body’s one chance to set the Sertoli-cell population — and so the lifetime ceiling on fertility — before the long childhood quiet. It is also a diagnostic window: a brief period when the axis is awake and its function can be measured directly.
